Creating Successful Diabetes Self-Management Plans for Your Patients   by Halle Elbling

PROGRAM DETAILS

Diabetes is the fifth-deadliest disease in the United States and is the 7th leading cause of death. Diabetes also doubles one's risk of heart disease and stroke—leading to even more fatalities. Furthermore, poorly controlled diabetes, commonly a result of improper disease management, inadequate self-care and/or poor nutritional choices, leads to multiple other complications that are often very serious. In fact, in addition to cardiac effects, poorly controlled diabetes is the leading cause of adult blindness, end-stage renal disease and non-traumatic lower limb amputations. And, with over 23.6 million Americans with diabetes (7.8% of the U.S. population) and an overall economic cost of over $174 billion annually, this disease is obviously one of the major health dilemmas of our time.

Fortunately, however, people with diabetes can take control of their disease and lower their risk of complications through healthy lifestyle choices and preventative self-care techniques. As health care providers, we must combine the most current empirical knowledge in nutritional therapies with medication management and behavioral change strategies in order to reach the optimum results with each patient. Education on self-management of diabetes must include providing patients with easily implemented, yet impactful, plans for proactive diabetes control. This comprehensive, nutrition-focused seminar speaks to attendees from various professions and backgrounds that treat patients with diabetes. While the focus is on therapeutic nutrition and lifestyle modifications, the course provides key information for effective diabetes therapy, from medications to foot care, and increases your confidence in your ability to manage the disease. Upon completion of this program, you will be able to empower your patients to make informed choices that could not only improve their quality of life, but also extend it.

ABOUT THE PRESENTER

HALLE ELBLING, MS, RD, a Registered Dietitian and diabetes educator, has been working in the field of nutrition for more than 13 years. For the past four years, Halle has held the position of Outpatient Education Coordinator for the Palomar Pomerado Health System, Diabetes Health Department in San Diego County. She is an active member of the American Association of Diabetes Educators, American Dietetic Association, California Dietetic Association and the Society for Nutrition Education. Her past experience includes teaching Human Nutrition at San Jose State University, supervising child nutrition services for a childcare organization as well as the South Bay Union school district and serving as the dietitian and assistant manager for a local WIC program. Halle holds a Master of Science degree in Nutritional Science from San Jose State University, where she received the distinction of Outstanding Master's Student for the Department of Nutrition and Food Science. She earned her bachelor's degree in Nutritional Science from California Polytechnic State University. Possessing a vast amount of knowledge in nutrition, wellness and diabetes education for children and adults, Halle is committed to improving the health of others by providing comprehensive nutrition and health education through seminars for other professionals and individual counseling to patients and their families.
